Eligibility Criteria:

This offer may be available to members who already have an existing government subsidized OPIP Health plan with OMA Insurance, and:

• Have not previously been declined or downgraded from Health Plus under policy 17884 or 50131
• Are not on OPIP Health Continuance
• Do not have a pending application for Health Plus under policy 17884 or 50131
• Do not already have existing Health Plus under policy 17884 or 50131
• Live and work in Ontario or Canada and work in a province we are registered to operate and do business in (we are not registered or licensed in Quebec or the Territories, members living in these provinces with existing OPIP cannot participate in this upgrade offer).

How to Upgrade your coverage:

1. Visit omainsurance.com/OPIP for details on OPIP Health Plus coverage or contact us at 1-888-596-8881, option 2 to speak with OMA Insurance sales team. A licensed insurance staff will support your application process
2. Complete the upgrade form
3. Your new OPIP Health Plus coverage will be effective the first of the month following receipt of this form
4. Any premium payments required for coverage upgrades will be deducted from your current payment method on file with the plan administrator

Your upgrade form must be received by March 3, 2025.
